Professional Geriatric Care
Managers (PGCMs) are health and human services specialists who help families
care for older relatives, while encouraging as much independence as possible.
Over the years ACT NOW RNs has learned that many families need and want case
management services. In response to this need, ACT NOW RNs has added Debra
McPherson to our staff to handle all of your care management needs on a fee for
service basis. Debra is a member of the
National Association of Professional
Geriatric Care Managers, the Case
Management Society of America, the National Guardianship Association and
the Arizona Fiduciary Association. She is a certified fiduciary and
can assist you will all of your care management needs.
What Can a Care Manager Do For You?
Assess the clients Health, Functional, Cognitive,
Psychosocial, Environmental, Financial and Legal Needs
Develop a Detailed Plan of Care to provide recommendations for
appropriate interventions, services and resources
Refer, Coordinate and Monitor in-home care and other services
Act as an advocate during hospital stays
Arrange and attend Physician appointments and help guide the
client through the health care system
Review and Monitor the Medical, Legal and Financial needs of
the client, and make referrals to Specialists and other
Professionals
Review
and Monitor the clients Health Status, treatments and therapies to prevent
costly crises and unnecessary
hospitalizations
Function as a Liaison between family members, Health care
professional and other service providers to promote communication, understanding
and support
Educate and assist the client and family to promote
Empowerment and Involvement in the decision making process
Provide Certified Fiduciary and Medical Health Care Power of Attorney services
Who Needs A Care Manager?
People
who have no family members or whose family members are out of town, or long
distance care givers
People
who are in an urgent need of coordinated services such as medication management,
home health care, transportation or other services
Trust Officers and banks who have clients that desire a
Nursing Professional for coordination of care
Attorneys who have clients who desire a Professional to
represent them as Medical Health Care Power of Attorney
People
who desire to have a Nursing Professional act as an Advocate during hospital
stays
People who desire to have a Nursing Professional review and
monitor on an ongoing basis their Health status, medications and treatments
